Discharge Planning Coordinator - Home Tracheostomy and Ventilation Clinic

BC Children's Hospital

Vancouver, BC

The Discharge Planning Coordinator (DPC) is responsible for coordinating, supporting, and facilitating comprehensive discharge planning activities for patients and families. This includes family education, support, acquisition of home medical equipment, family meetings, care conferences, and referrals to community services and follow-up care. The DPC develops and maintains educational materials and collaborates with the interdisciplinary healthcare team, family advisors, and educators. They work closely with the Patient Flow Coordinator and other team members to support patient flow, ensuring timely discharges and transfers. The DPC also leads quality improvement activities, facilitates discharge processes, and manages family experience surveys and discharge data.


What you'll do

  1. Support bedside nurses and healthcare team in discharge planning for NICU and MBC patients, including equipment acquisition, referrals, and transition to community care.
  2. Develop individualized family learning plans to support discharge readiness, collaborating with the healthcare team.
  3. Support family-centered care by providing education, developing materials, and mentoring nurses.
  4. Engage families in the discharge planning process, advocating for their involvement in care decisions.
  5. Identify high-risk patients and develop strategies for successful discharge.
  6. Communicate regularly with healthcare team and community services regarding discharge plans, especially for complex cases.
  7. Facilitate family conferences and discharge planning meetings as needed.

What you bring

Qualifications

  • Graduation from an approved School of Nursing with current registration with BCCNM.
  • At least three years of recent neonatal nursing experience or equivalent.
  • Current CPR and NRP certifications.
  • Indigenous Cultural Competency Training.
  • Knowledge of social justice frameworks, trauma-informed care, and understanding of Indigenous health disparities and systemic racism.

  • Knowledge of social determinants of health and legislative commitments related to Indigenous rights and anti-racism.

Job Details

Type: Temporary, Full-Time until June 1, 2026

Wage: $49.20 - $67.08/hour

Location: 4500 Oak Street, Vancouver BC

Hours: Monday-Friday, 08:00-16:00
