Job summary

Winnipeg Regional Health Authority (WRHA) is seeking a Hospital Based Case Coordinator to assess eligibility for Home Care supports and develop care plans. Working with the client, family, and interdisciplinary team, you will coordinate discharge planning and determine if care is best provided in a personal care home or alternate environment.

The role requires RN certification (CRNM) and substantial nursing experience, with strong communication skills and proficiency in Procura and interRAI-HC

Qualifications

• Minimum three years recent nursing experience.
• Hospital-based case coordination experience is an asset.
• Graduate of an approved Registered Nursing education program.
• CRNM registration is required.

Responsibilities

• Assess eligibility for Home Care program supports.
• Coordinate plan of care with client, family and team.
• Assist in discharge planning from acute care settings.
• Liaise with local health care services, hospitals and families.
