Qualifications

• RN with active license (Florida) and knowledge of discharge planning requirements.
• Experience in direct care management and nursing leadership.
• Familiarity with CMS Conditions of Participation for discharge planning.

Responsibilities

• Supervises discharge planning and social services in inpatient, observation, and ER.
• Implements corporate care management strategies with facility management.
• Participates in multidisciplinary rounds for effective discharge planning and barrier resolution.
• Provides care management expertise to the interdisciplinary team.
• Uses reports to monitor documentation accuracy and regulatory compliance.
• Facilitates care conferences for complex transitions and placements as needed.
