Job description

Reporting to the Executive Vice-President, Quality, Professional Practice, Patient Experience & Chief Nursing Executive, the newly created role of Director, Seniors Care will play a pivotal role in leading the development of a Seniors Care Strategy to foster a culture of compassionate and high-quality care for older adults throughout their journey across Southlake Health and beyond.


This will involve strategic leadership for a number of seniors care services, such as the ACE, TCU, RCU-MCC, RCU units, the Behavior Support Team and Inpatient Geriatric Consults to ensure integrated care delivery.


The Director of Seniors Care will lead the charge in centralizing leadership and aligning senior care programs, creating an integrated approach that enhances the overall care experience for older adults. This initiative will lead to improved care coordination, ensuring seamless transitions between hospital, post-acute, and community services, ultimately reducing readmissions and length of stay.


By implementing evidence-based care models tailored for seniors and frail patients, the goal of the Seniors Care Program is to achieve better patient outcomes. Additionally, this strategy will focus on resource optimization, minimizing unnecessary acute care utilization while maximizing the efficient use of resources across departments. By strengthening the continuum of care, we will enhance integration between the hospital and community, providing comprehensive support for seniors throughout their care journey.
