Job description

Queen Square Family Health Team (QSFHT) provides accessible, equitable, patient-centred primary care to its local community. A talented, interdisciplinary team works collaboratively with Queen Square Doctors, and together, we serve 35,000 patients annually. We offer a wide range of programs and services including diabetes care, seniors’ wellness, palliative care support, mental health, maternal and children’s health and gender affirming health. QSFHT is a dynamic organization meeting the community’s needs during the recent pandemic. We are a founding member of the Central West Ontario Health Team. We are proud of our continuing role in the health system as active seekers and participants in collaborations and partnerships that serve the diverse needs of our patients, clients, and team members.

We are seeking a dynamic and results-driven Director of Expansion Operations with experience in the healthcare sector. Conditional upon receiving funding for our expansion, the ideal candidate will oversee the expansion of our family health team as part of the provinces commitment to attach every Ontarian to a family doctor. This individual will report directly to the CEO at Queen Square FHT. While some of the work can be completed virtually, it will be expected to work in person a minimum of 2-3 days per week.

Position Overview

The Director of Expansion Operations plays a pivotal leadership role within the QSFHT, responsible for ensuring the effective, efficient, and strategic operation of all clinical and administrative functions within our expanded team. This individual also co-leads the organization’s strategic growth initiatives, fostering partnerships, expanding services, and optimizing healthcare delivery within the community in alignment with provincial healthcare priorities and Ontario Health mandates.

Key Responsibilities

Operational Leadership

Oversee day-to-day operations of the expanded Family Health Team, ensuring seamless coordination across clinical, administrative, and support functions.

Implement and maintain quality improvement and performance measurement systems to enhance patient care and operational efficiency.

Develop and manage the operational budget, ensuring cost-effective use of resources and compliance with funding requirements.

Lead and mentor interdisciplinary team members including nurse practitioners, allied health professionals, and administrative staff.

Strategic Planning & Expansion

Collaborate with the CEO and Board to develop and implement strategic plans for organizational growth and service expansion.

Identify and pursue opportunities to expand programs and services that meet evolving patient and community health needs.

Lead feasibility studies, project planning, and implementation of expansion initiatives including new sites, partnerships, or service deliveries.

Represent the organization in external collaborations with Ontario Health Teams (OHTs), community partners, and other healthcare stakeholders.

Stakeholder Engagement & Communication

Foster a culture of collaboration and continuous improvement across teams.

Engage with patients, staff, community agencies, and government bodies to align services with community health priorities.

Communicate regularly with funders, including the Ministry of Health and Ontario Health, ensuring accountability and transparency in reporting and operations.

Human Resources & Organizational Development

Support recruitment, onboarding, training, and development of staff to maintain a high-performing team.

Develop and implement policies that promote a healthy, inclusive, and safe workplace culture.

Manage performance, support conflict resolution, and promote staff retention and satisfaction.

Qualifications

Bachelor’s or Master’s degree in Health Administration, Public Health, Business Administration, or a related field. Master’s degree preferred.

Minimum of 3 years of progressive leadership experience in a healthcare setting, ideally in primary care, community health, or multidisciplinary team-based environments.

Strong knowledge of Ontario’s healthcare system, including Family Health Teams, Ontario Health, and funding/accountability frameworks.

Demonstrated experience in operational management, strategic planning, and leading organizational growth.

Exceptional interpersonal, communication, presentation and leadership skills.

Proven ability to manage complex projects, budgets, and human resources.

Experience with quality improvement methodologies and data-driven decision-making.

Preferred Assets

Familiarity with EMRs (e.g., Accuro, PS Suite, Oscar), scheduling systems, and healthcare analytics.

Experience working with or within Ontario Health Teams.

Knowledge of equity, diversity, and inclusion practices in healthcare delivery.
