Role Profile – Director of Care

Mission

Reporting to the Executive Director, the Director of Care is responsible for leading, organizing and managing nursing and care services within the home and maintaining best practice through the provision of Resident centered quality care in accordance with provincial legislation.

The Director of Care works collaboratively within the interdisciplinary team and oversees all clinical operations including monitoring and evaluating nursing practices, to lead and support continuous quality improvement measures to ensure targets are achieved.

Key Responsibilities

To maintain the nursing department, monitor and evaluate all clinical programs and services to ensure they comply with legislative and professional standards.

To liaison with hospital, medical staff, community and health care related disciplines and partner to ensure the most effective delivery of care in the home.

Oversee risk management and quality assurance to uphold Resident safety.

Ensures the maintenance of accurate and up to date Resident health records, clinical assessments, and documentation,

To ensure the availability and maintenance of Nursing Department supplies and equipment to meet the departmental needs.

To oversee the financial and human resource management aspects of the Nursing department.

Develop, review and update job routines of all clinical staff. Provide ongoing support and leadership to clinical leads and all other clinical staff.

Promote a culture of engaged employees, achieving an overall increase in employee engagement versus prior year.

Lead clinical practice and quality meetings in the home.

Promote and provide Resident centered care consistent with Resident Bill of Rights and Governing Principles.

Ensure the implementation of continuous quality improvement initiatives, quality outcomes reviewed, and action plans developed to address indicators above benchmark target.

Maintain or exceed department annual budget.

Facilitate and promote the interdisciplinary approach within the home through the Nursing, Dietary, Activation, Environmental Service and Administrative Departments.

Co-ordinate and monitor the Infection Control Program for the Nursing Department

Review all resident applications for admission to determine appropriate level of care; assist with processing of admission and discharge papers.

Implement and monitor control and safekeeping of drugs and narcotics; supervise the drug administration program including the administration of medication by all registered staff.

Establish and maintain positive relationships between Residents, families, staff, volunteers, and the community.

Participate in Care Conferences as appropriate, meet with residents / family to resolve problems / concerns / complaints.

Hire, orient, schedule, supervise and evaluate the performance of nursing staff including use of disciplinary process.

Provide training and coaching and develop improvement plans for nursing staff as required.

Working with the department of nursing staff to ensure required clinical programs are in place, monitored, and evaluated against appropriate standards to ensure they align with legislative requirements and best practices.

Monitor and review resident care staff compliance with all facility policies and procedures, including safety rules and safe working practices.

Comply with all relevant Provincial and Federal Acts and Regulations.

Assist the Executive Director with budget preparation and implementation for the Nursing Department.

Participate in the CARF accreditation process as applicable.

Follow and adhere to Revera’s Guiding Principles.

Adheres to responsibilities and requirements for reporting adverse resident occurrences subject to regulatory and legislative direction.

All other duties as assigned.

Required Education And Experience

Minimum of one (1) year of experience working as a Registered Nurse in the long-term care sector.

Minimum of three (3) years of experience working as a Registered Nurse in a managerial or supervisory capacity in a health care setting.

Current registration as a Registered Nurse with the applicable Provincial licensing body required.

Bachelor or Master of Nursing preferred.

Current CPR certification required.

Demonstrated leadership and communication skills.

Professional competencies related to life-long learning, public relations, and health environment awareness.

Possession of strong motivational, team building and time management skills.

Must be computer literate in Microsoft Office Suite.

Proficiency in a second language is an asset.
