Partners Community Health (PCH) is a new not-for-profit organization, focused on bringing healthcare services together around the needs of people living in Mississauga and West Toronto. PCH currently operates two new state-of-the-art LTC homes and community hub in West Mississauga. The new homes have a combined 632-beds and be part of PCH’s larger strategy that introduces innovative and inclusive programs and services and new models of care delivery. Position Summary:

PCH is seeking a Director of Care to support the Partners Community Health team. Reporting to the Executive Director, the Director of Care is a critical role within the home ensuring that the home operates efficiently and delivers high-quality care to the residents. As a Director of Care, in partnership with the Executive Director, you will lead a team of clinical professionals to build relationships with residents and their family members, offering a variety of support options and programs to enrich their daily health and well-being. As a leader and part of a wider team, you are a strong contributor to the culture of the organization, where the focus is always about resident care.

Key Responsibilities:

  1. Monitor and ensure clinical care processes are delivered according to individual plans of care for residents.
  2. Lead, implement, and monitor key clinical programs, such as falls prevention, skin and wound care, continence care, and pain management.
  3. Meet regulatory compliance by ensuring the required company policies, provincial regulations, and applicable long-term care legislations are applied on a daily basis.
  4. Promote a resident-centered care approach that engages residents, families, and team members to deliver care that meets individual resident needs.
  5. Promote a positive working environment for team members and external providers.
  6. Manage and monitor all nursing envelope expenditures, explain monthly variances, and work with the Executive Director to identify corrective actions.
  7. Participate in strategic planning and accreditation activities.
  8. Establish a standard of practices within the nursing team to ensure that policies and procedures are followed, and Nursing Practice Standards are met in accordance with College of Nurses Standards.
  9. Establish, monitor, and evaluate staffing patterns and work schedules in accordance with the nursing envelope budget and with Collective Bargaining Agreements, as applicable.
  10. Collaborate with Nursing Leaders on learning needs and make provisions for appropriate education and continuing team member development.
  11. Be responsible for the effective delivery in the evaluation of the mandatory clinical programs and Quality committees such as Resident Safety, Leadership & Quality, and Professional Advisory Committee.
  12. Work in accordance with the organization’s Health and Safety Policies and Procedures and in compliance with the Occupational Health and Safety Act.
  13. Perform other duties as assigned.

Qualifications:

  1. Bachelor of Nursing Science or equivalent post-diploma education as an asset.
  2. Gerontological Nurse Certification (GNC) an asset.
  3. Current registration as a Registered Nurse with the provincial College of Nurses.
  4. Must have a minimum of three years of experience working as a Registered Nurse in the long-term care sector (ON).
  5. Must have a minimum of 3 years’ experience working as a Registered Nurse in a managerial or supervisory capacity in the healthcare setting (ON).
  6. Must have demonstrated leadership and customer service.
  7. Strong oral and written communication skills to include computerized documentation systems.
