Job summary

A health care organization in Sudbury is seeking a full-time Dietitian to provide nutritional services and support patient health. Ideal candidates will be Registered Dietitians with experience in community care and must be proficient in both English and French. The role offers a competitive hourly rate of $43.29 to $50.00 as per the OPSEU Collective Agreement.

What will you do?

· Respond to referrals from Care Coordinators to properly administernutritional and dietetic services to patients.

· Assess patients to identify their nutritional needs and determine patient-centeredtreatment goals.

· Provide treatment/nutritional care program and develop resources toaddress patient dietary needs and work towards achieving therapeutic goals.

· Oversee patient progress and adjust treatment programs when needed, toensure nutrition goals are addressed.

· Educate and counsel patients, their families, caregivers, and/or SDMs,regarding nutritional conditions/diseases, patient issues/concerns, and dietarystrategies to improve function, compensate for difficulties, and promotenutritional health.

· Manage service deliveries in conjunction with the Care Coordinator, theirfamilies, caregivers, and/or SDMs using all available forms of communication.

· Link patients with other in-house services or outside organizations.

· Balancepatient needs and choices with available resources, ensuring patients’ valuesand preferences are respected.

Accuratelydocument and chart patient’s assessment results, goals, treatment plans, andinterventions.

What must youhave?

· Registered Dietitian in good standing with theCollege of Dietitians of Ontario (CDO).

· Relevant experience providing nutrition anddietetic services in a community/health care environment.

· Comprehensive knowledge of dietetic services,practice methods, professional skills, nutrition principles and strategies.

· Strong interpersonal and collaboration skills tomanage service delivery in conjunction with Care Coordinators, patients, theirfamily members/caregivers, and other health care professionals.

· Good understanding of the roles of other healthcare professionals and complementary therapies.

· Knowledge of funding agencies available to assistpatients in accessing needed support services and other community resources,treatment centers, and organizations.

· Practical knowledge and understanding of pertinentlegislation.

· Comprehensive knowledge of the standards ofpractice and professional guidelines set forth by CDO.

· Ability to prioritize professional duties andmanage caseload in a time efficient manner.

· Ability to accurately complete requireddocumentation, reports, forms, and recommendations.

· Effective listening, observation, and facilitationskills.

· Ability to communicate information effectivelythrough a variety of means including meetings, reports, letters, andpresentations.

· Working knowledge of computer software (email,internet) and Microsoft Office applications (Word, Excel).

· Must have valid driver’s license and access to avehicle.

· Advanced oral and written proficiency in Englishand French is essential.

· Casemanagement experience or recent related community experience

· Abilityto speak French or another second language
