Job summary

Southbridge-Care-Homes in St. Catharines, Ontario is seeking a Registered Dietitian for our Food and Nutrition Department. You will conduct nutrition assessments, design individualized care plans, and collaborate with the interdisciplinary team to support resident well-being.

Responsibilities include utilizing RAI MDS 2.0, approving menus, and educating residents and families. ACDO membership eligibility and geriatric nutrition expertise are required for this permanent part-time role.

Qualifications

  • Registered Dietitian, eligible for membership with the College of Dietitians of Ontario.
  • Extensive knowledge of geriatric nutrition.
  • 3-5 years’ experience in long-term care setting preferred.
  • Proficiency with computer applications, Microsoft Office, and the internet.
  • Experience with RAI MDS 2.0 desirable, familiarity with software PointClickCare an asset.
  • Excellent organizational, time management, and multi-tasking skills.
  • Knowledge of Occupational Health & Safety practices, principles, and legislation.
  • Excellent oral and written communication skills are essential.
  • Customer-focused attitude, with demonstrated ability to build and maintain effective team relationships with staff and residents.
  • A negative vulnerable sector check completed within 6 months of hire date.
  • Completion of a 2 Step TB Skin test within the last 12 months.

Responsibilities

  • As a member of the Clinical Team, the Dietitian consults with and advises the Food Service Manager and Director of Care concerning nutrition and hydration program needs.
  • Completes nutritional assessments for each resident using RAI MDS 2.0 as required.
  • Determine an individualized resident’s diet and other nutritional interventions to meet his or her needs by involving the resident, family members and care team.
  • Develop, implement and evaluate nutritional care plans that include identification of nutritional concerns, goals and interventions for all residents.
  • Maintain excellent communication with the Food Service Manager regarding the changing needs of the residents.
  • Review the two seasonal menus for nutritional adequacy and provides approval in writing.
  • Educate residents, family members, and team members regarding nutrition and hydration recommendations.
  • Act as a member of the Interdisciplinary Team, acts as a representative for Southbridge, through participation in regional and professional activities as appropriate.
  • Participate in interdisciplinary committees related to resident care, facility operation and Quality indicators as required.

Southbridge, the owner and operator of several long-term care homes and retirement communities in Ontario, is currently seeking a Registered Dietitian to become part of a dynamic food and Nutrition Department. You will ensure that our residents are provided with nutritious and delicious meals, that are prepared and served with love.
