Job summary

LMC Pharmacy in Brampton, Ontario, is seeking a Pharmacist to join a collaborative, patient-centered team. This full-time role focuses on diabetes care, education, and chronic disease management within LMC Healthcare clinics.

You will work closely with endocrinologists, diabetes educators, and other professionals to deliver evidence-based care while maintaining strong patient relationships both in person and over the phone.

Qualifications

• OCP Part A licensed pharmacist in good standing with current injection certification.
• 2–3 years of Ontario pharmacy practice, with diabetes education experience preferred.
• Retail pharmacy experience required and willingness to travel locally to LMC sites.

Responsibilities

• Provide diabetes counseling, education, and medication management for patients.
• Deliver personalized education on diabetes management, medications, lifestyle, and adherence.
• Collaborate with prescribers and healthcare team to ensure coordinated care.
• Document consultations and follow-ups in the EMR, staying current with guidelines.
