Trinity Ward is a 25-bed Nurse Led Community Hospital ward. It mainly provides rehabilitation beds for elderly care following trauma/surgery and non-acute medical/minor illnesses. We also offer palliative care beds for those reaching the end of life or in crisis at home, ensuring dignified holistic care for patients and their families in a calm and caring environment.
We promote patient involvement and empower patients and family members to provide input into their clinical care and decisions. We respect that each individual has the right to autonomy and independence and endeavor to promote that independence as able.
An excellent opportunity has arisen for a motivated, dedicated individual to join this established and hardworking team. You will be responsible for delivering the highest quality of care to a diverse client group predominantly managed within a nurse-led facility. You will utilize highly developed clinical skills including diagnostic skills, alongside experience in long-term condition management and elderly care.
The role requires effective communication skills and the necessary leadership abilities to manage a multidisciplinary team and assist in case managing a group of patients from admission to discharge.
