Job summary

The Education & Support Coordinator at Alzheimer Society KFL&A will develop, coordinate, and deliver educational programs and support services for people living with dementia and their families. This part-time, 21-hour weekly role is based in Kingston, with responsibilities spanning individual and group support, workshops, and community outreach.

You will work with healthcare providers, volunteers, and community partners to connect clients with resources and services while promoting dementia

Qualifications

  • Post-secondary degree or diploma in Social Work, Gerontology, Nursing, Health Sciences, Public Health, Adult Education, Human Services, or a related field.
  • Dementia Studies or Gerontology certifications are assets.

Responsibilities

  • Provide information, education, emotional support, and resource navigation to individuals living with dementia and their families.
  • Conduct client assessments to identify needs and develop individualized support plans.
  • Facilitate referrals to community services and healthcare resources.
  • Lead support groups and caregiver education programs.
  • Maintain confidential client records and service documentation.
