Wabano provides a holistic model of care for Indigenous youth and seeks an experienced front-line youth worker who would like to move to the next step in their career by coordinating culture, case management and health promotion. The Wabano Centre for Aboriginal Health is an award-winning Health Access Centre that provides holistic health services to First Nation, Inuit and Métis people living in Ottawa – one of the fastest growing and diverse urban Indigenous communities in Canada. The Role

The Culture as Care Coordinator/Youth Team Lead will guide the youth team in their roles of culture, case management, and health promotion and be responsible for program targets and deadlines while also assuming a hands-on role in supporting youth 16-24 to transition from care of CAS or formal customary care.

To meet these responsibilities, the Culture as Care Coordinator/Youth Team Lead will:

  • Liaise with CAS in northern communities, Ottawa, and other organizations such as the Boys and Girls Club, ROH;
  • Build and maintain relationships with community partners such as CAS and group homes;
  • Coordinate and facilitate Indigenous integrated plans of care based on the model of care;
  • Conduct program evaluation and reporting;
  • Advocate for social justice and policy change;
  • Provide an integrated strength-based programming approach;
  • Plan, prepare, deliver and evaluate land-based programs;
  • Lead and support team members by completing team and individual work plans, meeting goals, reporting, and problem-solving;
  • Source consultants (e.g., Elders, traditional educators, facilitators), manage contracting of consultants for youth events planning and program delivery;
  • Provide orientation for staff and consultants;
  • Provide case management and systems navigation to eligible youth and manage referrals from Indigenous and mainstream organizations.
Qualifications
  • Masters or Bachelors degree in Social Work or other applicable human service program;
  • Clinical supervision experience preferred;
  • 3-5 years of experience in Case Management;
  • Experience with youth programming and effective communication with Indigenous youth;
  • Knowledge of Indigenous people’s history and challenges Indigenous youth face in contemporary society;
  • Demonstrated superior problem-solving ability with sound judgment, and strong oral, and written communication skills;
  • Demonstrated experience working with high-risk youth and managing crisis situations;
  • Knowledge of available resources in Ottawa and ability to research the resources available in remote communities outside of Ottawa;
  • Knowledge of on- and off-reserve governance and structures;
  • Strong organizational and negotiating skills and the ability to handle sensitive issues with tact and diplomacy;
  • Ability to work a flexible work week that may include non-traditional hours, evenings, and weekends.

Range $50,000 – $55,000 per year + Benefits
