Job summary

CMHA Peel Dufferin invites applications for an Immediate Intervention Specialist to join the Black Health and Social Services MDT. This on-site, permanent role focuses on crisis assessment, immediate intervention, and connection to services within a BAC community hub.

The successful candidate will assess needs, coordinate care, and support group activities while collaborating with other specialists to meet regional demand and equity goals.

Qualifications

• Minimum 2 years of experience in mental health and addictions.
• Experience with assessment, brief interventions and short-term supports is required.
• Fluency in additional languages is an asset.

Responsibilities

• Act as low barrier first response and front-line resource to the community.
• Assess and provide crisis support; connect to crisis services when appropriate.
• Assess client eligibility for services with a stepped care approach.
• Determine appropriate interventions and service intensity.
• Provide short-term task-based interventions to alleviate distress.
• Offer single session 1:1 in person or via phone for immediate support.
• Facilitate CMHA PD groups and assist with calendar coordination.
• Communicate interventions and care plans with primary care providers.
