About the HART Hub
The London HART Hub is a low barrier recovery-oriented space supporting individuals experiencing homelessness with mental health and addiction needs. It provides access to evidence-informed, respectful, trauma-informed, culturally appropriate, and compassionate care, connecting individuals to a continuum of services including transitional supportive housing, healthcare, and mental health and addiction services in one location.
Role of Counsellors
Counsellors will develop and maintain therapeutic relationships, provide addiction treatment planning, increase client motivation, and support wellness and bio-psycho-social-spiritual health.
What We Offer
- Starting wage: $27.14/hour (Level 1 – Direct Service), with a differential based on regulation ($30.01/hour for regulated professionals)
- Internal candidates will be placed as per the Collective Agreement
- Work schedule: 70 hours bi-weekly, rotating days and afternoons, including weekends
- Benefits: 9% in lieu of benefits
- Pension: Immediate eligibility for HOOPP
Key Responsibilities
Intake, Screening, Crisis
- Participate in intake services, utilizing evidence-informed screening tools
- Respond effectively to crises, providing intervention and de-escalation
- Adapt services to meet clients’ social and cultural needs
- Assess transportation needs and provide assistance
Assessment
- Use standardized assessment tools to determine suitability and care level
- Interpret assessment results and develop client-centered treatment plans
- Reassess regularly to monitor progress and adjust plans
Counselling and Treatment
- Build therapeutic relationships, uphold ethical standards
- Utilize motivational interviewing and stages of change models
- Apply evidence-informed counselling techniques
- Develop and update formal treatment plans collaboratively with clients
Case Management
- Match clients with appropriate services based on assessments
- Manage caseloads, meet service targets
- Facilitate community treatment groups as needed
Service Coordination/Navigation and Advocacy
- Support clients in navigating services, advocate on their behalf
- Lead client-centered case conferences
- Coordinate with social justice system involved clients
Information and Referral
- Provide support and education on addiction and mental health services
- Connect clients rapidly to housing and support services
- Engage in outreach, community development, and education
Supervision
- Participate in clinical and administrative supervision
- Provide peer support
Qualifications
Mandatory:
- Post-secondary education in a related field (e.g., BSW, diploma in addictions)
- Registration with a professional college (e.g., OCSWSSW, CNO, CRPO, CPSO, COTO, CPO)
- Valid driver’s license and access to transportation
- Current Police Vulnerable Sector Check
- Ability to attend work regularly
Preferred:
- Additional training (e.g., ASSIST, crisis intervention)
- Experience with substance use treatment and homeless populations
- Knowledge of mental health, legal system, and Ontario’s London Hub model
- Bilingual in both official languages (French and English) preferred
