JOB SUMMARY
Reporting to the Manager, the Care Coordinator is a key member of the interdisciplinary team. Using clinical triage protocols, technology platforms, critical thinking, communication tools and other resources, the triage nurse identifies the most appropriate care path for patients of the program. Assisting the physician in the clinical setting can be the main responsibility when in a remote clinical setting.
