About the Opportunity

The Care Coordinator assesses, plans, authorizes services, and manages cases for clients referred to Continuing Care. They facilitate community-based services, support hospital discharges, and work within budgets and policies. The role involves collaboration with community and hospital teams, advocating for clients, and maintaining communication with all care providers.

Responsibilities
  • Assess client needs and develop care plans.
  • Authorize services and make referrals to community resources.
  • Support clients in the community and during hospital discharge processes.
  • Work within budgets and policies, advocating for clients when necessary.
  • Follow up with clients during transition periods to ensure successful community reintegration.
Qualifications
  • Bachelor's Degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies.
  • Registration or eligibility for registration with Nova Scotia Regulatory bodies.
  • At least 3 years of community-based care management experience.
  • Knowledge of community agencies, resources, and case management.
  • Excellent assessment, communication, and negotiation skills.
  • Ability to work independently and in a team.
  • Valid driver's license and access to a vehicle.
  • Additional languages, especially French, are a plus.
Hours and Compensation
  • Full-time, long-term assignment, approximately one year.
  • 75 hours bi-weekly, with shifts including days, evenings, weekends, and holidays.
  • Hourly rate: $44.22 - $51.61.
