The Niagara Falls Community Health Centre (NFCHC) is a non-profit, community-governed organization dedicated to providing accessible, comprehensive health services at no cost to residents of Niagara Falls. NFCHC offers a wide range of programs including primary care, dental services, mental health counseling, nutrition guidance, occupational therapy, and specialized support for 2SLGBTQ+ individuals, refugees, and newcomers. The Centre emphasizes health promotion and community development through free workshops and outreach initiatives designed to address social determinants of health. Position Summary:
Reporting to the Clinical Director the Complex Care Case Worker will provide a broad range of support services to individuals including administrative and social support, case management, system navigation and advocacy services. The incumbent will lead care coordination for complex clients in consultation with internal and external supports with a focus on patient-centred care. In addition, this position will be responsible to assist with the design and operation of services at NFCHC including maintaining quality assurances. The Case Worker will work with Primary Care Providers to assist in supporting their most complex patients providing input and consulting on cases as well as developing and facilitating wrap around supports that enhance the patient’s sense of health and wellbeing.
Main Responsibilities/Duties:
- Provide care coordination and supports to clients referred internally and externally utilizing a variety of methods.
- Assist Clinical Team with expedient completion of a variety of assessments and forms (i.e. ODSP)
- Assist with the Client Concern process by ensuring there is follow-up and agreed upon actions are completed.
- Maintain client records with timely and accurate reporting.
- Provide guidance and direction to other CHC counsellors on caseloads and overall counselling protocol.
- Assist clients to meet identified needs and actively lead and participate in their care plans. This includes referring clients to other social services or supporting clients in utilizing community resources including legal, medical care, financial assistance, housing, employment, transportation, day care, and other services.
- Ensure confidentiality within the bounds of legislation.
- Act as a client advocate on individual and systemic issues.
- Work within the guidelines of their regulatory college.
- Contribute to the on-going evaluation, development and implementation of the therapeutic supports that respond to the psycho-social health needs of the NFCHC clientele.
- Liaise with community agencies regarding issues relevant to client needs and identify and develop partnership/collaboration opportunities (when applicable).
- Provide input for development of team and NFCHC programming and services.
- Develop and maintain strong ties to other professionals and community members.
- Work in a manner that demonstrates commitment to NFCHC’s mission, vision, and values.
- Contributes to the development and implementation of NFCHC’s strategic goals and objectives.
- Participate in professional development opportunities as identified in the performance management process.
- Participate in quality improvement activities as identified by the NFCHC.
- Adhere to the expectations of NFCHC policies and procedures and operating procedures.
- Document, record, and report on activities in an accurate and timely manner.
- Work in a safe manner as per relevant health and safety legislation and NFCHC policies.
- Responsible for professional liability insurance.
- Any other reasonable duties as required.
Education and Employment Requirements:
- Certification and membership in good standing within a regulatory college (i.e. Social Work, Occupational Therapy, Nursing, etc.)
- Working knowledge of coordination of client care
- Experience in working in a community health setting would be an asset
- Certifications relevant to target clientele (addictions, acute and complex care) also an asset
- Experience working with people who are marginalized, socially isolated, living in poverty, and with complex physical and mental health needs, including newcomers and refugees, senior citizens, youth, individuals and families with low incomes, and people experiencing homelessness.
- Excellent clinical, communication, and problem-solving skills
- Proficiency in the use of EMR systems to follow established protocols and maintain accurate and timely documentation
- Knowledgeable with resources within the community and methods of access
- Maintaining an understanding of the health issues of the community to provide appropriate services
- Proven ability to provide compassionate, respectful, non-judgmental and culturally competent care to the disenfranchised and primary populations
- Demonstrated communication, time management, problem solving, and organizational skills
- Previous experience establishing collaborative working relationships with a multi-disciplinary team, clients, and community service providers
