As a Complex Care Clinician (RN/RPN) - Preventative Complex Care Team you will:

  • Apply specialized theory, clinical expertise, and knowledge relating to designated/specific client populations, working across multiple sites/clinics/services (if applicable).
  • Leverage clinical expertise and knowledge to address the complex medical, cognitive, and behavioral needs of eligible clients, primarily focusing on delivering safe, ethical, and high-quality care.
  • Ensure that each client's care plan is tailored to their specific needs and goals, actively integrating principles of prevention, chronic disease management, palliative care, and self-care management as needed.
  • Provide wrap-around care to designated client populations, with a strong commitment to client and family-centered care.
  • Perform ongoing assessments to identify issues or deviations from the care plan, collaborating with medical providers, care providers, interdisciplinary team members, and community agencies.
  • Collaborate with interdisciplinary team members, leadership, and other healthcare professionals to enhance quality of life and achieve excellence in client and family-centered care.
  • Maintain dedication to individualized and comprehensive care.
  • Contribute to the overall well-being and positive outcomes of clients.
About Home Health Programs

Home Health programs provide health care and support services to people with acute, chronic, palliative, or rehabilitative needs, often with ongoing health issues and complex personal/social needs, requiring care planning and resource coordination. Clients include frail elderly, younger clients with disabilities, and adults with chronic illnesses needing support to maintain independence.

The Client Population

Home Health clients typically have ongoing health issues and/or complex personal/social issues, requiring care planning and coordination of appropriate resources. Clients include frail elderly, younger clients with disabilities, and adults with chronic illnesses requiring supports to maintain or promote independence.

Qualifications

Education & Experience

  • Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Two years’ recent case management or home care nursing experience, including work with the complex medically frail population or an equivalent combination of education, training, and experience.
  • Valid BC Driver’s License required for local area travel, using personal vehicle.
Knowledge & Abilities
  • Knowledge and ability in nursing practice related to Home Health Preventative Complex Care clients.
  • Proficiency in comprehensive, evidence-informed, and strength-based holistic health assessments and evaluating health promotion activities, using multiple sources and methods.
  • Knowledge of assessment and screening tools (e.g., risk assessment tools, Inter-RAI, ethical decision tools, Zarit tool, MMSE).
  • Ability to perform focused assessments, synthesize information, and develop client-centered care plans based on evidence and best practices.
  • Critical thinking and clinical judgment skills to prioritize care during unexpected changes.
  • Ability to engage clients and involved individuals as active partners in care planning and decision-making.
  • Competency in monitoring, evaluating, and adjusting care plans based on effectiveness and changes.
  • Skills to connect with clients to promote shared decision-making, considering determinants of health.
  • Ability to organize and facilitate meetings with clients and care teams.
  • Proficiency in basic and advanced nursing skills (e.g., medication administration, wound care, vital signs, pain management, delegation).
  • Knowledge of disease management, palliative care, mental health, and substance use assessments and interventions.
  • Skill in conducting goals of care conversations.
  • Ability to provide culturally safe care, including for Indigenous populations, in accordance with standards.
  • Understanding of Trauma-Informed Practice and Harm Reduction.
  • Knowledge of relevant provincial acts, regulations, and policies.
  • Proficiency in autonomous scope of practice and standards for RN/RPN.
  • Skill in CPR techniques.
