• Led the care team with a clear shift report, mentoring PSWs and aligning daily priorities.
• Assessed, planned and delivered care for residents with complex needs with dignity and safety.
• Administered medications safely and monitored for effectiveness and side effects.
• Documented care plans and used technology to keep information current and accessible.
