Job description

Under the direct supervision of the Health Manager, the Community Wellness Coordinator plays a vital role in supporting the overall health and well-being of Sheguiandah First Nation community members. This position requires a deep understanding of Indigenous culture, traditions, and values, and commitment to promoting holistic Wellness.

Responsibilities

  1. Build strong relationships with community members of all ages.
  2. Identify and address community health and Wellness needs through needs assessments and surveys.
  3. Develop and implement culturally appropriate health promotion programs and initiatives.
  4. Facilitate community workshops, support groups, and cultural events.
  5. Collaborate with community leaders, elders, and other stakeholders.

Wellness Services

  1. Provide individual and group counseling and support services.
  2. Offer crisis intervention and support.
  3. Assist with assessing health care and social services.
  4. Promote mental health, substance abuse prevention, and healthy lifestyles.
  5. Facilitate traditional healing practices and ceremonies.

Advocacy and Referral

  1. Advocate for the health and well-being of community members.
  2. Connect individuals to appropriate resources and services.
  3. Collaborate with external agencies and organizations.

Data Collection and Reporting

  1. Maintain accurate and confidential client records.
  2. Collect and analyze data to monitor program effectiveness.
  3. Prepare reports and presentations as required.

Qualifications

  1. Strong understanding of Indigenous culture, traditions, and values.
  2. Post-secondary education in social work, health promotion, or a related field preferred.
  3. Experience working with Indigenous communities.
  4. Excellent interpersonal and communication skills.
  5. Ability to work independently and as part of a team.
  6. Strong organizational and time management skills.
