Work Hours: 35 hours/week Mon-Fri 9am-5pm

Work Setting: Exclusively Onsite at 650 Queen St E, Toronto, Ontario

Salary: External Rate – I7: $59,282.34

Program Overview - Community Supports and Outreach (CSO)

The WoodGreen Community Supports and Outreach program serves clients 18 years old and older with mental health challenges and addictions who have experienced homelessness. This holistic program also offers mobile community case management supports, community development with recreational programming, and hoarding and pest management. The clients live in four WoodGreen owned small-scale apartment buildings, as well as independent units throughout the City of Toronto.

What You Will Do

  • Develops, implements, and evaluates the Healthy Communities - Healthy Futures program, which promotes wellness through educational workshops, nutritional demonstrations, health promotion clinics at community venues (libraries, supportive housing sites, elderly person centers, etc.).
  • Perform initial and ongoing needs assessment from a strengths-based client support approach.
  • Provide case management services including care planning, assessments, personal goal setting, short- and long-term planning, supportive counselling, follow up supports, crisis intervention, care and service coordination, case conferencing and referrals as needed.
  • Cooperate with Housing staff and support tenants/clients with strategies to maintain housing, as well as transition planning to support move ins and outs as required.
  • Collaborate with Housing staff to support tenants/clients with issues pertaining to their unit such as pest control, unit damages, clutter/hoarding, and/or concerns related to fire & safety.
  • Provide crisis prevention, management, and intervention support including conflict mediation, interpersonal supports, and assistance in illness management.
  • Establish and maintain an ongoing flexible and supportive relationship with clients.
  • Introduce new tenants to other tenants and staff and to the various committees and activities they can participate in.
  • Assist tenant/client to develop a supportive network.
  • Provide support and/or assistance, if necessary, when tenant moves in/out of their unit.
  • Keep documentation and client files current and updated and collect data as required.
  • Document any significant problems, events, changes to services, special needs, etc.
  • Apply ethical standards related to the provision of community supports.
  • Work in collaboration with other WoodGreen staff to ensure the program operates efficiently and effectively.
  • Participate and represent the organization at external committees and forums, etc.
  • And other duties as assigned.
  • This position is subject to exposure to second-hand smoke, odors, pests and clients’ potential use of inappropriate language or swearing.
What You Bring to the Team
  • Bachelor’s degree in Bachelor of Social Work (BSW) or Related Degree.
  • Registered Social Worker (RSW) in good standing with the Ontario College of Social Workers and Social Service Workers is an asset.
  • Minimum 3 years of solid community experience.
  • Related experience in case management, supportive counselling, conflict/crisis mediation and client engagement.
  • Demonstrated experience working with individuals with mental health, substance use/addiction issues, physical health issues, and/or street involvement.
  • Ability to communicate in American Sign Language (ASL) considered a strong asset.
