COMMUNITY SUPPORT WORKER

About the role

As our Community Support Worker, you will work as part of a vibrant team to provide care and support to people living in staffed homes within their community.

You’ll encourage and empower the people we support to be independent and self-determining, enabling them to live the life they envision while maintaining excellent standards of care, health, and safety.

Your natural ability to care for others, along with your capacity to work independently and collaboratively within a multidisciplinary team, will contribute to your team’s success.

Core Responsibilities
  1. Respect and advocate for the supported persons’ rights and choices as needed.
  2. Interact effectively with the people we support.
  3. Empower the people we support to be active members of their community.
  4. Participate in developing, implementing, and documenting Personal Service Plans.
  5. Foster positive relationships with family members, professionals, co-workers, and community members.
  6. Assist with activities of daily living and medication administration.
  7. Apply behaviour management principles and strategies, including addressing mental health needs.
  8. Record daily activities and personal health information (PHI) in our case management database.
  9. Ensure the home is clean and well-maintained, including daily food preparation and light housekeeping.
Qualifications
  • Grade 12 education or equivalent.
  • Community Support Worker Diploma or similar qualification.
  • Minimum of 2 years experience in community social services or healthcare.
  • Current Emergency First Aid / CPR and Food Safe certificates.
  • Previous behaviour management training.
  • Effective verbal and written communication skills.
  • Comfort with full personal care.
  • Class 4 driver’s license preferred, Class 5 at minimum.
  • Satisfactory driver’s abstract.
  • Criminal record clearance for children and vulnerable adults.
  • Negative TB test.
  • Moderate computer literacy.
  • Eligibility to work in Canada.
