Job summary

Sage Seniors Association in Edmonton seeks a Specialized Link Worker for the ALC Community Transitions Program. The role provides direct, person-centered support by connecting older adults to programs and services to address needs in their care plan and to navigate available supports.

This permanent, full-time in-person position (35 hours/week) offers a salary of $40,586 to $48,504 per year, with 100% employer-paid Health & Dental and a 5% matched Pension.

Qualifications

  • Knowledge of issues impacting older adults and transitions from ALC to community resources.
  • Proficient problem-solving and advocacy skills.
  • Computer literacy including Microsoft Office.
  • Strong planning, organizational, and documentation skills.
  • Police Information Check including Vulnerable Sector is required.
  • Vehicle with valid class 5 license and $2M liability insurance.
  • Experience with older adults with complex needs preferred.
  • Experience with diverse cultures is an asset.

Responsibilities

Specialized Link Worker

ALC Community Transitions Program

The Specialized Link Worker will work with Individuals referred into the program by Acute care. The Specialized Link Worker provides direct, person-centered, individualized support by connecting older adults to programs and services that address social and practical needs identified in their care plan. This includes educating individuals about available supports, assisting them in accessing, navigating and connecting to services and ensure services are delivered. The goal of this position is to support older adults to transition home from ALC spaces to age safely in place by coordinating necessary supports.

QUALIFICATIONS, SKILLS & EXPERIENCE
  • Knowledge of issues impacting older adults including complex needs and transitions from ALC settings and community resources available

  • Proficient planning, organizational and documentation skills

  • Vehicle required; valid class 5 driver’s license and $2 million auto liability insurance
  • Experience working with older adults with complex needs is preferred
  • Experience working with diverse cultures and ethnocultural communities is an asset

This position is a permanent, full-time in-person (35 hours/week) position. The salary range for this position is $40,586 - $48,504 plus 100% Employer paid Health & Dental plan, and 5% matched Pension plan. Salary considerations are based on an assessment of the successful candidate’s education, skills, previous experience and other factors. Sage Seniors Association offers an outstanding work life balance with 3 weeks of vacation to start, a 35-hour workweek, and generous sick leave accruals. Sage also observes 13 statutory holidays in the calendar year. This position will require some evenings and weekends; shift differential will be applied.

Pay: $40,586.00-$48,504.00 per year
