Job summary

Carecor Health Services Ltd. is seeking a dedicated Community Personal Support Worker for the Grand Bend area, providing in-home care and support to clients in rural Ontario.

You will assist with daily living activities, health monitoring, light housekeeping, and meal prep while following individualized care plans. The role requires PSW credentials or enrollment in a nursing pathway, with CPR/BLS, CPI preferred, and immunizations up to date.

Qualifications

Carecor is seeking dedicated and compassionate Community Personal Support Workers to join our community care team. In this role, you will travel to clients’ homes to provide high‑quality personal care and support that empowers individuals to live safely and independently in their own environment.

As a Personal Support Worker with Carecor, you will assist clients with daily living activities including bathing, grooming, dressing, mobility, toileting, and medication reminders. You will also provide light housekeeping, meal preparation, companionship, and emotional support. This position is ideal for PSWs who are confident working independently, adaptable to varied client needs, and committed to delivering dignified, person‑centered care.

You will follow individualized care plans, maintain accurate documentation, and communicate key observations to supervisors or care coordinators. Availability: Standard working hours are Days (6am – 2pm) and Evenings (2pm -10pm). Employees need to include every other weekend (2 weekends a month).

Key Responsibilities

• Provide personal care service according to directions on the established care plan or assignment sheet, and ensure that, on an ongoing basis, the emotional, spiritual, physical comfort and safety of clients are met to the greatest possible extent
• Provide personal care that is within the provincial scope of practice for a Personal Support Worker; duties may include skin care, hair care, mouth care, bathing, bowel and bladder care, positioning and movement, exercise, basic wound care, feeding including special diets, and assistance with oxygen administration and medication reminder.
• Perform specialized, client specific procedures for which recognized training and current competence can be demonstrated.
• Observe and document client conditions according to company standards, report unusual events or changes in the client’s condition to the immediate Supervisor or to the Office
• Understands and implements infection prevention practices
• Participate in ongoing internal and/or external continuing education activities.
• Adhere to Bayshore Policies and Procedures.
• Participate in quality activities and continuous improvement initiatives in keeping with the company's Quality Management System.
• Participates in proactive Health & Safety activities while performing all duties and is responsible to notify immediate Supervisor of any Health & Safety risks or concerns.
• Maintains confidentiality of client and corporate information and discusses same only with appropriate Bayshore personnel.
• Complete other tasks as requested.

Job Qualifications

• Personal Support Worker (PSW) certificate or enrolled in a Nursing pathway program or a Nursing student
• CPI Certification required (Crisis Prevention & Intervention Training Certificate) - preferred
• Clinical experience as a PSW within the past 2 years
• Current Basic Life Support (BLS) certificationand N95 Mast Fit Test Certificate
• Proof of current immunization for measles, mumps, rubella, and varicella (MMRV), Hepatitis B, and Tdap/Td
• Proof of negative TB skin test within the past(1)yearor negative chest x-ray within the past 5 years
• Willing to undergo Vulnerable Sector Police Check as a condition ofhire
