Qualifications

• 3 years of recent pre-hospital care experience.
• Experience as a Community Paramedic.
• Ability to communicate in multiple languages.

Responsibilities

• Provide proactive client care within residential, community and non-traditional settings.
• Conduct assessment, clinical planning and case management for referred clients.
• Liaise with primary health care providers, home care services and allied health agencies to improve access to services.
• Identify gaps in current service provision and promote access to community health services.
