about the role:

Sherbourne Health and The Neighbourhood Organisation co-lead the Health Access St. James Town initiative, a collaboration of 15 health and social service providers. The Health Access Expansion Project will focus on expanding the existing Health Access St. James Town model to surrounding neighbourhoods.

This initiative aims to create and improve access to health care and other community support services by coordinating and aligning with client health care needs, including equitable pandemic response and sustainable healthcare response to residents, specifically those who are facing multiple barriers to health care and social supports, with a focus on black and indigenous populations.

The Community Outreach Worker will be mainly responsible for community engagement and outreach initiatives. In addition, they play a key role in neighbourhood initiatives and dissemination of resources to communities.

Responsibilities and Tasks:

  • Create and implement outreach and community engagement plans.
  • Develop community resource mapping and provide information across agencies and staff.
  • Plan and conduct workshops, focus groups, and community events to disseminate health-promoting best practices to the community and connect community members to resources.
  • Work closely with community ambassadors and plan, implement and direct community outreach and awareness programs.
  • Create promotional materials and flyers for events and provide event updates and flyers to communication teams.
  • Organize and conduct capacity building program for community ambassadors.
  • Participate in creating and directing special event activities.
  • Develop organizational partnerships to work with community engagement & Outreach staff with partner organizations.
  • Work with the Health Access Team and partners to plan and coordinate community engagement initiatives (e.g. walking tours, events) that promote knowledge of and celebration of the St. James Town neighborhood.
  • Strong training and facilitation skills.
  • Proficiency in the use of computers, including word and data processing, designing flyers and presentations; projectors, cellphones, phones, fax machines, printers, and photocopiers.

To thrive in this role, you’ll need

education/experience/skills/abilities:

  • Diploma in the health or social sciences or an equivalent of experience and expertise.
  • Ability to work within an anti-oppression framework; understand and appreciate the diversity of our client and staff population and community.
  • Ability to work within the harm reduction framework and support clients and knowledge and experience regarding harm reduction practices and knowledge of harm reduction strategies.
  • Experience working in diverse, urban neighborhoods.
  • Familiar with community-based services.
  • Experience working with high-need, hard-to-reach clients and communities.
  • Strong training and facilitation skills.
  • Excellent organizational and coordination skills.
  • Excellent interpersonal skills.
  • Second language is an asset.

working conditions:

  • The position will be located at St. James Town – The Corner, and other sites in the Downtown East.
  • The position will be expected to work evenings and weekend hours on a regular basis.
