A leading organization focused on healing and recovery is seeking an Outreach Worker to join their dedicated team. This role involves working closely with clients to provide support services and build meaningful relationships while operating within a community treatment model. Ideal candidates will have relevant experience and a deep understanding of Indigenous culture.

POSITION TYPE: PERMANENT, FULL-TIME (Mon – Fri; 8:45 am – 5:00 pm)
Functioning as part of the coordinated service delivery team, the Outreach Worker works proactively with new and existing clients, offering a range of support and recovery services within an existing service plan and on an as-needed basis. The Outreach Worker is a relationship-builder and advocate, providing services in a non-judgmental and culturally appropriate manner, helping clients build new skills and achieve their goals.
This program operates within an Assertive Community Treatment Model and utilizes Housing First Principles from a social justice orientation, including harm reduction, pro-choice, social determinants of health, human rights, social development, population health, health equality, and trauma-informed, client-centered practice.
Proven knowledge and ability in critical thinking, decision-making, problem-solving, and crisis management.
Demonstrated ability to prioritize and focus work-related activities to effectively achieve service goals and outcomes.
Commitment to the principles that guide Mount Carmel Clinic, as reflected in its history, vision, mission, and values.
Knowledge of Indigenous culture and traditions and understanding of the impact of colonization on Indigenous individuals and families.
Proven ability to work within an interdisciplinary team with respect, professionalism, sensitivity, and cultural awareness.
Strong knowledge and application of diversity, equity, cultural safety, pro-choice, social determinants of health, harm reduction, trauma-informed care, and community development principles.
Excellent verbal and written communication skills and proficiency in computer systems.
