Job summary

CarePartners is seeking RNs and RPNs to provide community nursing services in home and clinical settings. Positions available are full-time and part-time with weekdays, weekends, and on-call rotation.

Compensation based on a grid with progression by hours and role criteria; generous benefits, mileage reimbursement, and educational opportunities are offered. Travel is required with a valid driver's license.

Qualifications

• RN and/or RPN license with current CNO registration.
• Excellent English verbal and written communication.
• CPR certification and immunization requirements.
• Valid driver’s license and own vehicle for travel.
• Strong commitment to patient-centered home care.

Responsibilities

• Develop and maintain Care Plans for Community Health Services.
• Provide nursing services in-person and virtually to patients.
• Deliver services in home, clinic, retirement home, or school settings.
• Collaborate with interdisciplinary care teams and families.
• Perform additional clinical duties as required.

Wages

• For Registered Nurses (RN)- This position offers a competitive compensation grid, with progression based on hours worked and role-specific criteria. The RN per general visit range is $30.20 to $44.58 and the RN hourly range is $30.80 to $45.48. Placement within the grid will be determined in accordance with company policies and procedures, as well as the candidate’s knowledge, skills, qualifications, and relevant experience.
• For Registered Practical Nurses (RPN) - This position offers a competitive compensation grid, with progression based on hours worked and role-specific criteria. The RPN per general visit range is $24.07 to $29.60 and the RPN hourly range is $24.07 to $29.60. Placement within the grid will be determined in accordance with company policies and procedures, as well as the candidate’s knowledge, skills, qualifications, and relevant experience.

What The Role Involves

• Developing and maintaining an effective Care Plan with respect to Community Health Services, independently or in cooperation with the Ontario Health atHome.
• Working with patients (both in-person and virtually) to provide exceptional nursing services
• Providing services in a variety of settings which may include in-home, clinic, retirement home or school
• Communicating and liaising with patients, family members and others within the interdisciplinary care team
• Performing other clinical duties as required

What You Bring

• Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
• Current College of Nurses of Ontario (CNO) registration in good standing
• A strong desire and commitment to making a difference in the lives of our patients
• Exceptional interpersonal skills
• A valid driver’s license and vehicle as regular travel is required
• Excellent English verbal and written communication skills
• CPR Certification
• Current immunization record including 2 step TB test
• To protect our patients, we require a current vulnerable sector check and a clear background check
