This position supports patients' requiring additional care while they await the Long-Term Care plan or rehabilitation to home.

We are looking for RNs and RPNs who desire great flexibility in their schedules.

This is a Full-Time temporary 1 year position located in the Woodstock Transition Care Unit.

Hours of work are 7 x 12 hour shifts within a 2-week rotation inclusive of weekdays and weekends.

What The Role Involves

• Developing and maintaining an effective Care Plan with respect to Community Health Services, independently or in cooperation with the Ontario Health atHome.
• Working with patientsto provide exceptional nursing services
• Providing services in a variety of settings which may include in-home, clinic, retirement home or school
• Communicating and liaising with patients, family members and others within the interdisciplinary care team
• Performing other clinical duties as required

What You Bring

• Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
• Current College of Nurses of Ontario (CNO) registration in good standing
• A strong desire and commitment to making a difference in the lives of our patients
• Exceptional interpersonal skills

• CPR Certification
