Qualifications

  • Degree or diploma in nursing (RN or RPN).
  • Current registration with CNO in good standing.
  • CPR Certification and clear background check required.

Responsibilities

  • Develop and maintain effective Care Plans for Community Health Services.
  • Provide in-person and virtual nursing services to patients.
  • Communicate and liaise with patients and interdisciplinary care teams.

This is a Part-Time position in the Simcoe/Delhi area.

What The Role Involves
  • Developing and maintaining an effective Care Plan with respect to Community Health Services, independently or in cooperation with the Ontario Health atHome.
  • Working with patients (both in-person and virtually) to provide exceptional nursing services
  • Providing services in a variety of settings which may include in-home, clinic, retirement home or school
  • Communicating and liaising with patients, family members and others within the interdisciplinary care team
  • Performing other clinical duties as required
What You Bring
  • Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
  • Current College of Nurses of Ontario (CNO) registration in good standing
  • A strong desire and commitment to making a difference in the lives of our patients
  • Exceptional interpersonal skills
  • A valid driver’s license and vehicle as regular travel is required
  • Excellent English verbal and written communication skills
  • CPR Certification
  • Current immunization record including 2 step TB test
  • To protect our patients, we require a current vulnerable sector check and a clear background check

Through our Community Nursing Services outreach program, we’ve been organizing staff-led medical care and clinics in countries with poor access to health care since 2009.
