Job summary

CMHA Durham is seeking an experienced Registered Nurse to provide comprehensive primary care nursing within the HART Hub, serving marginalized clients with complex needs. You will participate in triage, assessments, and holistic care planning across mental health and addictions services.

The role emphasizes collaborative care within an inter-professional team, including medication administration and client education, with a flexible, on-site community clinic setting in Oshawa.

Qualifications

• Registered Nursing Diploma or Degree with at least two years in mental health.
• Ontario RN registration in good standing.
• First Aid/CPR required.
• Knowledge of Mental Health Act and related legislation.
• Ability to work in a multidisciplinary team.
• Strong communication and problem-solving skills.
• Valid Ontario driver’s license is an asset.

Responsibilities

• Provide full-scope nursing services as part of a multidisciplinary clinical team.
• Perform triage assessments and comprehensive mental health and addictions assessments.
• Deliver care, information and resources on prevention, chronic disease management and end-of-life planning to reduce harm.
• Administer medications and treatments, including injections, as required.
• Develop and implement care plans aligned with client goals within the HART Hub model.
• Document client interactions according to CMHA Durham policies.
• Support crisis response and collaborate with community services as needed.
