Job summary

Fraser Health’s nursing team seeks a Licensed Practical Nurse to provide comprehensive, holistic care in a community health setting. You will assess clients, plan and implement care, and collaborate with an interdisciplinary team to optimize outcomes.

Qualifications

• Grade 12 or higher with Practical Nursing program completion.
• Current practicing registration as a Licensed Practical Nurse with BCCNM and CPR Level C/First Aid.
• Valid BC Driver's License and access to personal vehicle.

Responsibilities

• Assess and monitor client strengths and needs across physiological, psychological, social and spiritual aspects.
• Contribute to care planning and attend interdisciplinary meetings.
• Plan and prioritize client care within scope of practice and guidelines.
• Teach and involve clients in care decisions and chronic disease management.
• Administer medications and provide reminders per policies.
• Document and update care information to ensure client safety.
• Assist with activities of daily living and support client independence.
