Are you an experienced registered practical nurse seeking a rewarding career that cares for others in a professional environment that cares for you? You’re in the right place.

As a Community Independence Care Coordinator (Full-Time; Immediate start; Location: Moosonee), you will assess and plan services for low-needs patients, focusing on patient independence through system navigation and community resource referrals. You will work with the Care Coordinator on complex cases and perform direct nursing tasks as needed.

Whether in our office, in the community, or at a healthcare facility, you will play a key role in providing connected, accessible, patient-centered care, supported by our team of over 9,000 healthcare and other professionals.

Responsibilities include:

  • Assessing care needs with patients and families, determining service eligibility, and developing care plans
  • Linking patients to service providers
  • Coordinating and monitoring care delivery
  • Building helping relationships with patients and families
  • Respecting patient values and preferences while balancing available resources

Minimum requirements:

  • Registered Practical Nurse in good standing with the College of Nurses of Ontario
  • Relevant healthcare and community support experience
  • Understanding of healthcare issues and priorities
  • Knowledge of healthcare tools, systems, and databases
  • Knowledge of community programs and relevant legislation
  • Strong planning, organization, and evaluation skills
  • Ability to complete documentation accurately
  • Proficiency in computer software and Microsoft Office
  • Excellent interpersonal and communication skills
  • Ability to work collaboratively with diverse groups and community agencies
  • Strong listening and facilitation skills
  • Effective communication of community service information
  • Fluent in English; French or other languages are an asset

Preferred qualifications:

  • Experience with diverse patient groups (multicultural, homeless, palliative, ABI, mental health, geriatrics, pediatrics)
  • Case management experience
  • Ability to speak French or another second language
