Job summary

Pacific Coast Community Resources is hiring a Community Inclusion Worker to support individuals in pursuing personal, social, vocational and volunteer activities within the community. You will promote independence, self-determination and safety while collaborating with care teams.

You will assist with planning, documenting and implementing the person’s goals and ensure rights are respected, with a focus on behavior management and mental health awareness.

Qualifications

• Must have strong communication with team members, professionals, families and community members.
• Experience in health care or community social services preferred.
• Criminal record clearance for children and vulnerable adults required.

Responsibilities

• Respect and advocate for the person’s rights and choices.
• Interact effectively with people we support.
• Empower the person to be a full citizen in the community.
• Coordinate participation in community-based activities.
• Foster relationships with family, professionals and community members.
• Participate in developing and documenting the Individual Service Plan.
• Record daily activities and health information in the case management system.
• Apply behavior management principles, including mental health awareness.
