LAMP Community Health Centre is a multi-service, community based charitable organization that provides a wide range of programs and services to improve a person's health. LAMP uses a co-ordinated approach to connect people to medical services and other programs that build personal resources. By providing services that improve physical, emotional, social and economic well-being, people can better control their health and environment.

Our organization is grounded in principles of health equity, and we are committed to delivering services and programs through an anti-racism, anti-oppression lens. We strive to create a healthier community by putting people first and addressing systemic barriers to care.

Under the supervision of the Youth Programs Manager, the Community Health Worker (Youth Focus) will develop and implement youth-focused outreach and health initiatives for youth ages 12–29 living in Central and South Etobicoke. The role supports youth and families by addressing social determinants of health, identifying risk factors, providing referrals and resources, advocating for clients, and participating in case coordination as appropriate.

The Community Health Worker will collaborate with interdisciplinary teams and community partners to strengthen service delivery, develop strategic partnerships, and implement innovative approaches to youth health and well-being.

The incumbent will ensure compliance with all organizational policies and procedures.

Primary Responsibilities:

Provide safe, supportive, and youth-centred services that promote health, wellness, and informed decision-making for youth ages 12–29 and their families.

Identify barriers and connect youth and families to appropriate health, social, and community services, resources, and supports.

Plan, implement, and evaluate youth programs, workshops, events, and activities focused on physical and mental health, healthy choices, education, leadership, and volunteer opportunities.

Facilitate age-appropriate group programs and workshops both on-site and in the community.

Conduct intake assessments and support service navigation across health, education, and social services.

Participate in case coordination, referrals, follow‑up, advocacy, and support for youth and families with complex needs.

Conduct community outreach and promote youth programs and health promotion initiatives through events, promotional materials, and digital platforms.

Collaborate with the Youth Programs team, interdisciplinary staff, and community partners to develop and deliver programs, initiatives, and special projects.

Develop and maintain strategic partnerships with community agencies and resources to address emerging needs, including violence, trauma, and other community‑identified challenges.

Develop and distribute health promotion resources in print and digital formats, including social media content and program materials.

Support risk assessments, conflict resolution, and mediation in collaboration with staff, management, and community partners.

Complete administrative duties, including registration, attendance, data entry, invoicing, petty cash, funder reporting, and grant support.

Participate in team meetings, case conferences, committees, and other activities that support service delivery and organizational goals.

Post‑secondary education in a relevant discipline (e.g., Social Work, Child and Youth Care, Social Service Work) and/or equivalent education and experience in health promotion or community development.

3–5 years of experience working with youth in diverse community settings, using anti‑oppressive, anti‑racist, harm reduction, and community‑based approaches.

Experience supporting equity‑deserving youth, including youth involved with the justice system, experiencing homelessness, or facing mental health challenges.

Strong experience in community outreach, youth engagement, program development, implementation, monitoring, and evaluation.

Excellent communication, interpersonal, group facilitation, conflict resolution, and relationship‑building skills.

Ability to work effectively with diverse populations, independently and as part of an interdisciplinary team.

Strong knowledge of community resources, public service systems, social supports, and client service navigation.

Ability to develop and maintain effective partnerships with community agencies and service providers.

Strong organizational and administrative skills, including client records, privacy requirements, social media, and data management.

Demonstrated professionalism, empathy, flexibility, initiative, dependability, and commitment to quality client service.

Ability to manage crisis situations and support clients through conflict in a calm, respectful manner.
