You would be working specifically within the Open Door Program (ODP), an initiative of the Mid-West Toronto Ontario Health Team (OHT) and led by Access Alliance. ODP is an exciting outreach-based health initiative aiming to improve health outcomes for systemically vulnerable populations in designated high priority neighborhoods in our region. ODP activities increase access to health education and services for individuals and communities. It provides preventative care and navigation/referrals to primary care, mental health and addiction services as well as needed community support services.

• Support planning, implementation and evaluation of community engagement programs/initiatives in accordance with the goals of the project;
• In conjunction with the ODP team, develop and roll out various outreach strategies in the community linked to identified needs;
• Develop and maintain both informal and formal community networks and partnerships and work effectively with community partners to implement collaborative programs and initiatives, and support capacity building;
• Assist in the promotion of programs, services and special events;
• Support individuals with developing preventive care plans;
• Plan, facilitate and implement information sessions, workshops, and group programs related to chronic disease prevention, management and other health topics as requested;
• Refer community members internally and externally as appropriate;
• Assist with the development of health education materials;
• Identify, compile and maintain lists of culturally and linguistically appropriate community services, programs and resources for use by clients, program participants, community members and staff;
• Develop and maintain health information resources and displays as appropriate based on ODP plans and communication strategy;
• Participate in and support all organizational systems and structures as required;
• Assist in program data collection, monitoring, evaluation and funder reporting;
• Submit program statistics and reports on a timely basis. Manage all expenditures and reporting as per organizational policies;
• Assist with program volunteer recruitment, orientation and training, as needed;
• Actively participate in the organization’s planning and evaluation activities;
• Develop an annual work plan in conjunction with your supervisor, identifying action plans, learning goals and professional development needs;
• Assist in other duties as assigned that are reasonable within the scope of the job.
• You need to have an undergraduate degree in Health Sciences, Social Work, Education or a combination of relevant education and work experience;
• Minimum 3 years of front-line experience in community-based work doing outreach, health education and group facilitation;

Preferred:

• Good knowledge of health promotion, health education and health issues affecting newcomer communities;
• Experienced in running programs and activities, organizing special events, coordinating engagement of other staff, volunteers, students and community members;
• Experience working with low-income, multi-racial, multi-lingual communities and interdisciplinary teams;
• Excellent interpersonal skills with colleagues and community members;
• Excellent communication skills, both written and verbal in English;
• Demonstrated flexibility and responsiveness to community needs;
• Excellent administrative, time-management and problem-solving skills;

Asset:

• Second language relevant to the populations served by the ODP.
• Hours: 35 hours per week
