You would be working specifically within the Open Door Program (ODP), an initiative of the Mid-West Toronto Ontario Health Team (OHT) and led by Access Alliance. ODP is an exciting outreach-based health initiative aiming to improve health outcomes for systemically vulnerable populations in designated high priority neighborhoods in our region. ODP activities increase access to health education and services for individuals and communities. It provides preventative care and navigation/referrals to primary care, mental health and addiction services as well as needed community support services.
