Alliance for Healthier Communities is seeking a Community Health Worker to support the Community Health Ambassador Model across Flemingdon Health Centre sites. You will advance health education, promote health equity, and help navigate systems for the community.
You will work closely with CHAs, partners, and a Steering Committee to strengthen partnerships and outcomes. The role emphasizes collaboration, community engagement, and program coordination, with evenings/weekends as needed.

Salary range: $56,547 to $67,252(along with HOOPP pension plan and benefits, depending on eligibility)

Flemingdon Health Centre (FHC) is a registered charity and not-for-profit Community Health Centre serving Flemingdon Park, Fairview, and Thorncliffe Park. Across three sites, FHC provides health-related services grounded in the social determinants of health, community engagement, equity, inclusion, accountability, excellence, collaboration, and partnerships.
The Community Health Worker supports the implementation and coordination of the Community Health Ambassador Model across Flemingdon Health Centre’s sites. As lead agency, FHC works in coordination with East Toronto Health Partners, North York Toronto Health Partners, partner agencies, a Steering Committee, and Community Health Ambassadors.This role supports equity-deserving communities in FHC’s catchment area by advancing health education, health promotion, and system navigation. The Community Health Worker works closely with CHAs at FHC and supports coordination of CHA activities across partnership sites.
