Alliance for Healthier Communities seeks a Chronic Disease & Mental Health Service Worker to join its multidisciplinary team at Flemingdon Health Centre in North York, ON. The role focuses on outreach, education, program coordination and data-driven improvement across multiple sites.

The position is a 6-month contract, part-time (0.6 FTE, 21 hours/week) with evenings and weekend hours possible, offering HOOPP pension and benefits eligibility.

Qualifications

• University degree and/or a combination of post-secondary education and experience

• Ability to speak another language is an asset
• Proficient in MS Office (Word, Excel, Outlook) and Canva; PS Suite experience is an asset

Salary range: $31.07 to $36.95 per hour (along with HOOPP pension plan and benefits, depending on eligibility)

Expected start date: ASAP

Expected end date: March 31, 2027

Locations: Flemingdon Health Centre at 10 Gateway Blvd, North York, ON (May occasionally rotate across other sites i.e. Fairview & Thorncliffe Park)

Background: Flemingdon Health Centre (FHC) is an engaged and involved member of some of Toronto’s most dynamic neighborhoods; Flemingdon Park, Fairview and Thorncliffe Park. Across our three sites, we provide a range of health-related services based on the social determinants of health with extensive community engagement.

The successful candidate will be a part of the Chronic Disease and Mental Health multidisciplinary team, connecting with and engaging community partners through environmental scanning and assessments, conducting extensive outreach, coordinating and supporting various community chronic disease-related programs and services as well as taking a lead in program evaluation and quality improvement initiatives.

Responsibilitiesinclude:

• Liaise and network with community agencies, leaders, and members around chronic disease prevention and management
• Outreach to various healthcare providers, internal and external
• Promote diabetes education and other chronic disease-related program, including tele-ophthalmology - in person, on social media, FHC’s website, in community events and fairs, to encourage referrals, recruit program participants and create general community awareness about FHC and the services we provide
• Support the delivery of chronic disease education sessions on/offsite
• Coordinate the full team’s chart audit, program evaluation and quality improvement initiatives
• Gather and organize and analyze data (including health equity), maintain accurate records, complete and submit regular progress and evaluation reports
• Address queries and answer phone calls and correspondence pertinent to the Chronic Disease services and programs
• Support the tracking of emergency food cards, transportation support and interpretation services across the full team
• Complete all administrative duties associated with this role
• Cross-collaborate with colleagues and providers within the organization
• Assist in the recruitment, training, supervision and engagement of volunteers
• Participate in internal/ external committees, as required
• Promote and support attainment of organizational vision, goals and objectives
• Other duties as directed by Manager, Chronic Disease and Mental Health or designate
• University degree and/or a combination of post-secondary education and experience
• Demonstrated community outreach and engagement skills
• Able to work with people from diverse communities
• Team player with demonstrated interpersonal skills; with high ability to exercise good judgment and tact
• Proven presentation, facilitation and organizational skills
• Proficient in MS Office (Word, Excel, PowerPoint, Publisher and Outlook) and Canva; knowledge and experience in PS Suite (electronic medical records system) an asset
• Proven capacity for initiative taking and ability to carry projects to completion with minimum supervision; ability to assess priorities and meet deadlines
• Ability to collect and analyze community data for the use of program promotion and development (e. g. statistics and health equity data)
• Demonstrated ability to access community resources: familiarity with the communities being served across the three sites: Flemingdon, Thorncliffe and Fairview
• Ability to speak another language other than English an asset
