Job description

Position Summary

In accordance with the vision, values, and strategic direction of the organization, the Community Health Worker II, as part of an interprofessional health care team and under the supervision of the Team Lead or designated clinical leader, provides home support services to clients according to a pre-established care plan. Responsibilities include assisting with activities of daily living such as bathing, grooming, oral hygiene, and meal preparation while promoting maximum independence. Performs delegated tasks for which training has been completed. A key aspect of this role is to ensure the well-being of individuals and families and to promote independent living within the community.

Must have current registration with the BC Care Aide Registry

Shift Rotation/Hours of work: Evenings, 16:00 to 22:30 (rotating)
