Position Summary

Under the guidance of the Team Lead or designated clinical leader, the Community Health Worker II, as part of an interprofessional healthcare team, provides home support services to clients according to a pre-established care plan. Responsibilities include assisting with activities of daily living such as bathing, grooming, oral hygiene, and meal preparation, while promoting maximum independence. Performs delegated tasks for which training has been completed. The core goal is to ensure the well-being of individuals and families and to promote independent living within the community.

Must have current registration with the BC Care Aide Registry.

Shift Rotation / Hours of Work: Fri-Tues, Fri-Mon, Evenings, 13:30 to 22:00 (rotating)

Location: Prince George

Qualifications:
