Job description

Island Health is hiring Community Health Workers to join our team on Vancouver Island. Multiple positions available, including full-time, part-time, casual and temporary.

As a member of a multidisciplinary health care team the Community Health Worker provides personal care assistance and support to clients and their caregivers/families to promote and maximize independence in the home setting, including assistance with activities of daily living and delegated tasks, as directed and in accordance with established plans of care.

Travel may be a requirement of this position. Transportation arrangements must meet the operational requirements of the Vancouver Island Health Authority in accordance with the service assignment and may require the use of a personal vehicle, and valid BC Driver's License.

Your role and responsibilities

  • Provides personal care to clients, including dressing, bathing, skin and hair care, feeding, transfers, and other tasks as assigned. Performs delegated tasks such as medication administration, catheter care, suppositories, applying non-sterile dressings, and participating in prescribed exercise and mobilization routines, all following specific training.
  • Monitors clients' physiological, socio-cultural, and general well-being, reporting any concerns to the supervisor. Encourages independence and self-sufficiency for clients and their caregivers/family, regularly reporting on their condition and care needs.
  • Demonstrates household management methods, providing information on activities of daily living, housekeeping, meal planning and preparation, and grocery shopping. Prepares and serves meals, including special diets, and assists or feeds clients as needed, in accordance with the Care Plan.
  • Maintains a safe and healthy environment by performing household duties like cleaning and laundry, as directed. Contributes to the development and delivery of client-centered Care Plans, documenting care provided and updating the supervisor on the client’s progress.
  • Ensures a safe working environment by following infection control procedures, removing hazards, reporting faulty equipment, accidents, injuries, and near misses, and adhering to safety protocols.

What you need to apply

  • Home Support/Resident Care Attendant/Health Care Assistant Certificate from a recognized post-secondary program; or an equivalent combination of education, training and experience.
  • Food Safe Certificate
  • Valid BC driver's license.
  • Current registration and in good standing with the BC Care Aide & Community Health Worker Registry.
