Job description

Island Health is hiring Community Health Workers to join our team on Vancouver Island. Multiple positions are available, including full-time, part-time, casual, and temporary roles.

As part of a multidisciplinary healthcare team, the Community Health Worker provides personal care assistance and support to clients and their caregivers/families to promote independence at home. This includes help with daily activities and delegated tasks, following established care plans.

Travel may be required for this position. Transportation arrangements must meet operational needs and may involve using a personal vehicle, along with a valid BC Driver's License.

Your Role and Responsibilities
  • Provide personal care such as dressing, bathing, skin and hair care, feeding, and transfers. Perform delegated tasks like medication administration, catheter care, suppositories, dressing changes, and participation in prescribed exercises, following specific training.
  • Monitor clients' well-being, report concerns, and encourage independence. Keep supervisors informed about clients' conditions and care needs.
  • Assist with household management, including activities of daily living, meal planning and preparation, grocery shopping, and providing meals, including for special diets. Support clients with feeding as needed, following the Care Plan.
  • Maintain a safe environment through household duties like cleaning and laundry. Contribute to developing and updating Care Plans and document care activities.
  • Follow infection control procedures, report hazards, faulty equipment, accidents, injuries, and near-misses, and adhere to safety protocols.
Minimum Requirements
  • Home Support/Resident Care Attendant/Health Care Assistant Certificate from a recognized post-secondary program, or equivalent education, training, and experience
  • Food Safe Certificate
  • Valid BC Driver's License
  • Current registration and good standing with the BC Care Aide & Community Health Worker Registry
