An established industry player in health care is seeking a dedicated Community Health Worker to join their multidisciplinary team. This role involves providing essential personal care assistance to clients and their families, ensuring they maintain independence in their home environment. As a member of a multidisciplinary health care team, the Community Health Worker provides personal care assistance and support to clients and their caregivers/families to promote and maximize independence in the home setting, including assistance with activities of daily living and delegated tasks, as directed and in accordance with established plans of care.
Travel may be a requirement of this position. Transportation arrangements must meet the operational requirements of the Vancouver Island Health Authority in accordance with the service assignment and may require the use of a personal vehicle; and valid BC Driver's License.
Home Support/Resident Care Attendant/Health Care Assistant Certificate from a recognized post-secondary program; or an equivalent combination of education, training and experience. Food Safe Certificate; and valid BC driver's license.
Current registration and in good standing with the BC Care Aide & Community Health Worker Registry.
