Job summary

Winnipeg Regional Health Authority is seeking an Integrated Program Wellness Coordinator within the My Health Team network. The role focuses on case management, collaboration with community clinics, and alignment of mental health strategies to support patient wellness.

The incumbent will report to the My Health Team Manager, work with inter-professional teams, and help deliver Recovery Model services across community and clinical settings.

Qualifications

• Post-secondary degree in one of the listed disciplines (BN, RPN, OT, PT, BSW, RT, SLP, RD) or an equivalent combination of education and experience.
• Registration with the relevant regulatory body is required.
• A valid driver’s license and vehicle are required.

Responsibilities

• Provide case management within the Recovery Model of care for patients where services are not available through existing providers.
• Coordinate with My Health Team partner clinics to align mental health strategies in the community area.
• Support and address wellness, health needs, and priorities of the patient population in liaison with partners and inter-professional teams.
