Qualifications

• Registered Social Worker with a bachelor’s degree and 2–4 years counseling experience.
• Master’s degree in Social Work preferred.
• Strong interpersonal and team collaboration skills.

Responsibilities

• Provide counseling to individuals and families within the primary health team.
• Offer crisis intervention and coordinate with mental health programs.
• Participate in case conferences and provide case management as needed.
• Maintain client records and ensure confidentiality in line with PHIPA.
• Lead or participate in mental health support groups and outreach activities.
