Job summary

WellFort Community Health Services in Mississauga, ON, is seeking a Registered Nurse (RN) or Registered Practical Nurse (RPN) for the Homeless Health Program. This full-time position supports primary care, chronic disease management, and health promotion for structurally vulnerable populations in Peel Region.

Qualifications

• Diploma or degree in nursing from a recognized program.
• Registration with the College of Nurses of Ontario in good standing.
• Experience in primary care or community health preferred.

Responsibilities

• Provide direct nursing care in primary care and homeless health settings.
• Perform nursing assessments and health education.
• Coordinate with multidisciplinary team to plan and evaluate programs.
• Ensure documentation in EMR and regulatory compliance.
• Participate in health promotion and advocacy activities.
