The Government of Northwest Territories seeks a Community Health Nurse to deliver critical nursing care within correctional facilities. The Community Health Nurse (CHN), Correctional Health Services, plays a critical role in delivering comprehensive, evidence-based nursing care within correctional facilities across the Northwest Territories (NWT). Operating under the Northwest Territories Health and Social Services Authority (NTHSSA), the CHN provides direct patient care that addresses the physical, emotional, psychological, and spiritual needs of individuals in custody, while promoting health, wellness, and continuity of care.
Working in facilities such as the North Slave Correctional Complex in Yellowknife, and correctional centres in Fort Smith and Hay River, the CHN functions both independently and collaboratively within an interdisciplinary team. Responsibilities include health assessments, chronic disease management, mental health evaluations, communicable disease prevention, medication administration, and response to medical emergencies. The CHN also facilitates preventive health initiatives, including immunization programs, STI screening and treatment, and health education tailored to client needs.
Given the high prevalence of complex health and social issues within the inmate population, the CHN provides care in a trauma-informed, culturally respectful manner and coordinates with external healthcare providers, specialists, and community organizations to support integrated care and safe transitions post-release.
The CHN contributes to system-level improvements through mentoring, participation in program development, and collaboration on research and policy initiatives. This role is essential in supporting the delivery of equitable, client-centered healthcare services within the correctional system and aligns with the broader mandate of the NTHSSA to ensure high-quality, standardized care across the territory.
Typically, the above qualifications would be attained by:
A Nursing Degree, and two (2) years of recent nursing experience in an ER setting or within a Community Health Centre or Public Health Unit.
