A local healthcare provider in Hamilton is seeking a Community Health Navigator to support individuals with complex health and social challenges. The role involves collaborating with healthcare professionals, providing individualized services, and engaging clients in improvement efforts for their quality of life. The ideal candidate has a strong background in social services and is committed to community well-being.
HOURS - The Community Health Navigator primarily will work a flexible daytime, Monday-Friday schedule. Occasional evening or weekend work may be required to respond to specific client needs
