Are you an RN/RPN who leads by example in meeting or exceeding clients’ expectations? Do you want to use your supervisory background to promote excellence in clinical service delivery? If so, you’re the type of professional who will excel as part of the CBI Health team.

As a Community Care Supervisor of Personal Support Services within our CBI Home Health team in Hamilton, you will lead a group of direct care providers to ensure that clinical service delivery meets or exceeds standards set by CBI Home Health/We Care and external stakeholders. You will develop and customize care plans to meet the patient’s individual needs, supervise direct care providers (field staff), unregulated staff (delegating tasks), or regulated staff (RPN), ensuring services align with their scope of practice, with a focus on service and clinical excellence.

PRIMARY DUTIES
  • Respond to patient referrals within timelines (internal and external).
  • Complete comprehensive care assessments promptly, utilizing tools, forms, and processes aligned with policy and procedure guidelines.
  • Document all required information pertinent to client status and care planning.
  • Ensure patient and care provider safety by accurately assessing risks using provided tools.
  • Ensure care plans meet individual patient needs and that care providers have the necessary knowledge, skills, and expertise.
  • Reassess patients during interim visits to ensure the service plan remains appropriate.
  • Investigate, resolve, and report customer-related complaints.
  • Promote service delivery excellence through effective onboarding, mentoring, coaching, and performance management of staff, including reviews.
  • Ensure team members meet performance expectations and align with role key accountabilities.
  • Participate actively in meeting or exceeding utilization, retention, and engagement goals.
  • Address staff training needs with action plans to resolve gaps and minimize risks.
  • Ensure all reporting meets standards and expectations of CBI Home Health/We Care, stakeholders, and regulatory bodies.
  • Complete documentation accurately and timely, including client charting.
  • Identify potential risks and collaborate with stakeholders to mitigate/manage risks, especially missed visits and clinical care issues.
  • Respond promptly and thoroughly to incident reports.
  • Enforce policies and procedures, ensuring they reflect best practices.
QUALIFICATIONS
  • Current registration with the College of Nurses of Ontario (CNO) as an RN or RPN.
  • Minimum of 2 years supervisory experience and/or experience in home health service delivery.
  • Ability to establish rapport effectively with clients and staff.
  • Valid Ontario driver’s license and access to reliable transportation within Hamilton and surrounding areas.
  • Willingness to participate in an on-call rotation.
  • Ability to develop effective solutions that align with policies and uphold best practices.
  • Self-motivated with the ability to work independently and as part of a team.
